How to change your profile photo's privacy settings:

If you only want your saved contacts to see your photos, select "My Contacts." If you don't want anyone to see your photos, you can limit visibility according to the available privacy options.

Hide photos from specific people.
A unique feature of Telegram is that you can set separate privacy rules for specific people. The Never Allow option lets you prevent selected contacts from viewing your profile photo.
