In light of the rising number of deaths from heart attacks in Uttar Pradesh and the serious problem of patients not receiving timely treatment, the state government has taken a landmark decision. Now, the life-saving injections Tenecteplase and Streptokinase will be administered free of charge to heart attack patients at all medical colleges, district hospitals, and major community health centers (CHCs) in the state.
Now no one will die due to lack of money.
This initiative is significant because this lifesaving injection costs ₹40,000 to ₹50,000 in the market, making it beyond the reach of poor or middle-class families. This decision ensures that no patient will lose their "golden hour" and die simply due to lack of money.
Why is this injection necessary in heart attack?
A heart attack occurs when a blood clot forms in the heart's arteries, blocking blood supply to the heart muscle. The first 90 minutes after a blockage is formed are the most dangerous, medically known as the "golden hour."
How the injection works

Tenecteplase and streptokinase are both clot-busting medications. They instantly dissolve clots and restore blood flow in the arteries. Experts say that if these injections are administered within the golden hour, mortality rates can be reduced by 30 to 40%. This decision will save thousands of lives, especially in rural and remote areas where cardiac hospitals are not readily available.
Treatment will reach every village through the hub and spoke model
Previously, these expensive injections and their treatment were limited to select super-specialty hospitals, often requiring patients to travel 30 to 40 kilometers to distant cities, wasting precious golden time. The UP government is now adopting a hub-and-spoke model to effectively implement this scheme:
Hub: Major super specialty hospital.
Spoke: District Hospital and Community Health Centre (CHC).
This model will ensure uninterrupted supply of medicines, training of doctors and monitoring of the entire scheme so that services can reach every village quickly.
Treatment protocol
The government has also issued a standard treatment protocol for this:
Immediate ECG: An electrocardiogram (ECG) will be done immediately upon arrival at the hospital.
Injection: Once a heart attack is confirmed, Tenecteplase or Streptokinase injection will be administered only by trained doctors.
Referral: After giving the injection, if required, the patient will be referred to a super specialty hospital for further specialized treatment.
This step is going to bring a big revolution in the health services of Uttar Pradesh.
